Add superelevation point type of "crown removed"

In the Superelevation Editor, adding a "crown removed" option to the Point Type list would help when trying to manually define/adjust anywhere there is 0% cross slope.

Allow for different shaft fixity in one model

A column on drilled shafts can have different fixity locations for different load cases. Suggest allowing analysis of the same bridge with different fixities rather then needing to create multiple models with varying fixities for simplicity.
